Decisions can be challenged without being silently rewritten.
An appeal links to the original decision, preserves history, and records the final reason.

Who may appeal
The affected organization, review author, reporter, or another person directly affected by a decision may appeal where identity and authority can be established proportionately.
What to provide
Include the decision reference, the disputed point, the policy or factual basis, and new supporting material if relevant. Avoid resubmitting identical information without explaining the requested reconsideration.
Review process
The appeal is reviewed against the methodology version and policy applicable to the original decision. A separate reviewer is preferred where operationally possible.
The outcome may confirm, amend, or reverse the decision. The reason and date are recorded; public history is updated without erasing the original event.
Urgent risk
Security threats, unlawful exposure of sensitive personal data, or credible immediate harm may receive temporary protective action while the full review continues.
